Clinical Pharmacist - Specialty/Rare DiseasePittsburgh, PA 15025 | $115,000-$140,000/year | Hybrid | Full-Time We are seeking a
Clinical Pharmacist to join a growing specialty pharmacy team focused on providing care for patients with
rare, orphan, and complex disease states. This is a clinically focused opportunity where you'll play an important role in optimizing medication therapy, educating patients, supporting access to specialized medications, and collaborating with providers and multidisciplinary care teams.
What You'll Do - Provide specialized clinical pharmacy services and oversee prescription dispensing for patients with rare and complex conditions.
- Review and verify medication orders, including identifying commercial versus free-drug prescriptions.
- Counsel and educate patients and caregivers on medication administration, adherence, potential adverse reactions, and therapy expectations.
- Receive and respond to patient calls, including adverse event reports and medication-related questions.
- Monitor patient progress and evaluate medication therapy effectiveness, making clinical recommendations when appropriate.
- Collaborate with physicians, nurses, and other healthcare professionals to support individualized treatment plans.
- Conduct occasional outbound outreach to physicians and other healthcare providers.
- Coordinate with manufacturers, insurance providers, and patient assistance foundations to help patients access necessary therapies.
- Assist with prior authorizations and coverage-related challenges to minimize treatment delays.
- Maintain accurate documentation of clinical interactions, counseling, adverse events, and therapy changes.
- Stay current on emerging therapies, clinical protocols, and developments in rare disease treatment.
- Provide education and training related to specialty and rare disease therapies.
- Oversee, train, and guide pharmacy technicians throughout order processing and fulfillment.
- Participate in scheduled after-hours/on-call clinical support.
What We're Looking For Required: - Doctor of Pharmacy (PharmD) from an accredited pharmacy school.
- Active, unencumbered Pennsylvania Pharmacist license.
- Strong verbal and written communication skills with the ability to effectively educate patients and communicate with healthcare providers.
- Ability to work rotating Monday-Friday shifts and participate in scheduled on-call responsibilities.
- Ability to follow a hybrid work model with approximately 50% onsite work.
Preferred: - Previous experience in a specialty pharmacy, particularly with rare, orphan, or complex disease states.
- Experience with CPR+ or CareTend pharmacy management systems.
- Experience supervising, training, or mentoring pharmacy technicians.
- Experience with patient counseling, adverse event intake, prior authorizations, or provider outreach.
Schedule & Training: This is a
hybrid position, typically structured as approximately
2 weeks onsite and 2 weeks working from home, although the structure may offer some flexibility based on business needs.
Training: Approximately
6-8 weeks, fully onsite before transitioning to the hybrid schedule.
The schedule will rotate between the following
Monday-Friday shifts:
- 8:00 AM-4:30 PM
- 9:30 AM-6:00 PM
- 11:30 AM-8:00 PM
Candidates interested in a
consistent 11:30 AM-8:00 PM schedule may also be considered.
On-Call: Pharmacists will typically participate in on-call coverage during weeks when they are scheduled for the late shift. An
additional on-call stipend is provided during assigned on-call weeks.
On-call needs are generally infrequent. If additional time is required to address an on-call situation, pharmacists are compensated for those hours in addition to the standard on-call stipend.
Specialty & Rare Disease Focus: The pharmacy supports medications and programs focused specifically on
rare and complex disease states, with new pharmacists supporting upcoming program launches. Therapies may include treatments for conditions such as
multiple sclerosis (MS), hereditary tyrosinemia type 1 (HT-1), and other rare diseases.
What to Expect Day to Day: This position offers a
blend of clinical pharmacy and dispensing responsibilities, with a greater emphasis on clinical work.
You can expect to:
- Review and verify prescriptions and orders.
- Determine appropriate free-drug versus commercial processing.
- Counsel and educate patients on their medications.
- Handle inbound patient calls and adverse event reports.
- Provide clinical guidance and medication-related support.
- Conduct occasional physician/provider outreach.
- Collaborate with internal teams and external healthcare professionals to coordinate patient care.
