URGENT HIRING: Clinical Manager (RN) - Broward County, FLActiva Home Health | Broward County, FloridaThe ideal candidate will have a strong background in home health clinical operations, proven leadership abilities, and comprehensive knowledge of Medicare home health regulations, OASIS assessments, clinical documentation, and episode management.
This is a key clinical leadership position responsible for overseeing daily clinical operations, supervising field clinicians, coordinating patient care, monitoring clinical performance, and ensuring compliance with all applicable state and federal home health regulations.
Key Responsibilities- Oversee and manage daily clinical operations to ensure the delivery of high-quality, patient-centered home health services.
- Supervise, guide, and support field clinicians, including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and other clinical personnel.
- Oversee patient admissions, Start of Care (SOC), Resumption of Care (ROC), recertifications, transfers, and discharge processes.
- Ensure timely and accurate completion of OASIS assessments, clinical documentation, and physician orders.
- Monitor patient episodes and ensure services are delivered in accordance with established plans of care and physician orders.
- Review clinical records for accuracy, completeness, and compliance with Medicare and Florida home health requirements.
- Oversee patient scheduling, case assignments, and coordination of clinical visits.
- Monitor clinical outcomes, patient satisfaction, quality measures, and overall staff performance.
- Provide ongoing clinical guidance, education, and support to field staff.
- Conduct chart audits and routine documentation reviews to identify opportunities for improvement.
- Assist with resolving clinical concerns, patient complaints, scheduling challenges, and operational issues.
- Ensure compliance with Medicare Conditions of Participation, Florida AHCA regulations, and applicable federal and state requirements.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
- Maintain ongoing survey readiness and assist with corrective action plans when necessary.
- Utilize Kinnser/WellSky for clinical documentation, scheduling, patient management, and episode tracking.
- Collaborate with the Administrator, Director of Professional Services, intake team, referral sources, and other members of leadership to support branch operations and organizational goals.
Required Qualifications- Active Registered Nurse (RN) license in the State of Florida.
- Previous experience as a Clinical Manager, Case Manager, or Clinical Supervisor in a Medicare-certified home health agency.
- Strong understanding of Medicare home health regulations and clinical operations.
- Demonstrated experience with OASIS assessments and home health episode management.
- Knowledge of Start of Care, Recertifications, Resumption of Care, Transfers, and Discharge processes.
- Kinnser/WellSky experience required.
- Strong understanding of clinical documentation, physician orders, and plan-of-care requirements.
- Knowledge of Florida AHCA regulations and Medicare Conditions of Participation.
- Excellent organizational, communication, leadership, and problem-solving skills.
- Ability to manage multiple priorities in a fast-paced home health environment.
- Strong leadership skills with the ability to motivate, develop, and hold clinical staff accountable.
Preferred Qualifications- Previous supervisory or clinical leadership experience in a Medicare-certified home health agency.
- Experience managing a multidisciplinary clinical team.
- Familiarity with OASIS-E documentation and current CMS requirements.
- Experience with quality assurance, chart audits, and survey preparation.
- Established knowledge of the Broward County healthcare community and local referral networks.
