Description of JobThe Work Unit: The Prevention Services Division (PSD) is actively building a workforce reflective of the diverse communities we serve. The Division strives to understand the needs of Colorado's communities and encourages contributions of stakeholders in identifying and solving public health problems. We are committed to providing a respectful, supportive and inclusive work environment that demonstrates our commitment to accept and respect individuals from all communities. The Division is also committed to the professional development, collaborative effort and sustained wellness of its workforce.
The Health Promotion and Chronic Disease Prevention (HPCDP) Branch in Prevention Services Division (PSD), reviews, plans, designs, implements and evaluates public health chronic disease interventions and related risk factors with the goal of advancing health and improving health-related quality of life for all Coloradans. Special emphasis is placed on evidence-based practices, data-based and evidence-informed decision making, capacity-building, partnerships, and integration between program efforts for efficiency and effectiveness. The statewide chronic disease programs within the HPCDP Branch include Heart Disease and Stroke Prevention, Comprehensive Cancer Control, Colorectal Cancer Control, Diabetes Prevention and Management, Alzheimer's and Related Dementias (ADRD), Healthy Eating and Active Living, the Cancer, Cardiovascular and Pulmonary Disease Grants Program and the Tobacco Education, Prevention and Cessation Grants Program.
We are aware that many chronic diseases share the same systemic causes: tobacco use, lack of access to nutritious foods, built environments to support physical activity, social connectedness, and poverty and economic instability. These systemic injustices and additional risk and protective factors are shared across many of the outcomes we address. Colorado For ALL means working toward a Colorado where everyone has the opportunity to succeed. It means we support the basic human rights of our neighbors regardless of gender, race, ethnicity, religion, national origin, age, sexual orientation, gender identity, citizenship status, education level, disability, socio-economic status, or any other visible or invisible identity.
This position is housed in the Diabetes, Cardiovascular Health and Alzheimer's Disease and Related Dementias Unit (CarDiADRD Unit) which focuses on engaging with health systems, community-based organizations and other partners to improve chronic disease outcomes for Coloradans, particularly those impacted by historical injustice and unequal distribution of resources. The unit also collaborates with internal programs including those focused on cancer, healthy eating and active living, tobacco, and informatics, among others, to support a comprehensive system of partnerships that address specific chronic disease needs in innovative and sustainable ways.
What You Will Do:This position is a work lead over the Chronic Disease Sustainability and Policy Coordinator (#02206) and the Chronic Disease QI Coordinator (#00995), providing strategic direction, oversight of work processes and products and assistance to these positions in accomplishment of their work.

The role supports the CarDiADRD Unit's implementation of chronic disease initiatives by leading the alignment of strategies across quality improvement, health policy, and sustainability. This position maintains a strong working knowledge of these content areas to ensure that strategies are cohesive, complementary, and effectively coordinated across team members and partners. This position also requires an understanding of how clinical quality improvement and workflow enhancements support the identification of individuals with chronic disease and connection to clinical and community-based services. It also applies knowledge of sustainability and policy approaches, including value-based payment models, reimbursement pathways, and health system alignment, to support regional and systems-level approaches that expand access to and utilization of chronic disease prevention and management services.
The position leads coordination of regional collaboration and consortium-based approaches that support shared service delivery models, strengthen community-clinical linkages, and improve efficiency and sustainability of programming, particularly in rural and frontier communities as part of Colorado's Rural Health Transformation Program. This includes working with health systems, community-based organizations, Regional Accountable Entities (RAEs), Accountable Care Organizations (ACOs), and state and academic partners to support integrated, coordinated care.
The role maintains a comprehensive understanding of strategies led across the team and ensures alignment across initiatives to reduce duplication, improve coordination, and maximize impact. The position also engages with statewide partners, including the Department of Health Care Policy and Financing (HCPF), the Governor's Office of eHealth Innovation (OeHI), the University of Colorado School of Medicine, and other partners, to support alignment between public health, health care delivery, and financing structures.
