Join The Salvation Army in the rewarding role of Youth Services Case Manager, where you will make a meaningful difference in the lives of runaway and homeless youth and young adults. As a Case Manager, you will provide individualized case management and social work services, including:
  • Crisis Intervention
  • Housing Support and Stabilization

Responsibilities

Job Responsibilities:
  • Conduct comprehensive client intakes, assessments, and evaluations. Develop individualized service plans and intervention strategies based on each client's needs, goals, and preferences.
  • Maintain regular contact with assigned clients and conduct home visits as needed to provide support, develop life skills, and promote self-sufficiency.
  • Provide therapeutic support, crisis intervention, and ongoing case management services.
  • Maintain accurate and timely case records, documentation, reports, and statistical data. Complete ongoing assessments and case recommendations.
  • Collaborate with service providers, families, and client-identified support systems to coordinate services, referrals, and resources.
  • Participate in case conferences, staff meetings, supervision, trainings, and collaborative meetings.
  • Build and maintain relationships with community agencies and represent The Salvation Army professionally in the community.
  • Assist with client interactions during street outreach and drop-in center activities.
  • Help maintain a clean, safe, and welcoming environment for clients.
  • Demonstrate cultural competence and support the goals and objectives of The Salvation Army Syracuse Area Services Strategic Plan.


Qualifications

Qualifications:
  • Bachelor's Degree preferred or Associates with at least three years of related experience.
  • Ability to work flexible schedule according to individual client needs, with some evenings and weekends required.
  • Knowledge of community resources desired.
  • Computer knowledge (Word/Excel) and ability to navigate Internet.
  • Access to private transportation during work hours and a valid driver license that meets The Salvation Army insurance requirements.
