Care Manager (RN) - Pediatric - Full Time

At NewYork-Presbyterian/Weill Cornell Medical Center, our Registered Nurse Care Managers are essential to delivering coordinated, high-quality care for complex patient populations. The RN Care Manager for the Pediatric service supports a highly vulnerable population requiring intensive, multidisciplinary coordination across the inpatient and outpatient continuum.

In this role, you will drive care coordination, promote efficient patient flow, and ensure safe, patient-centered transitions through proactive collaboration with the interdisciplinary team.

You'll manage complex cases and serve as a strong and knowledgeable liaison between acute care and critical care teams, coordinating the efforts of social workers, feeding specialists, PTs, OTs, cardiologists, intensivists and neonatologists - all in support of our patients and their families. Direct all aspects of inpatient care, ensuring optimal length of stay. Anticipate outpatient service needs, setting appointments and consultations prior to discharge.

Preferred Criteria
  • Case Management Certification (CCM) or willingness to obtain
  • Master's Degree in healthcare related field
  • Prior experience with Pediatrics
  • Prior experience in care management, utilization review, or discharge planning

Salary Range:

$134,900-$168,000/Annual
