What You'll Do - Job responsibilities
This Nurse Practitioner role splits time between direct patient care (two days/week) and provider performance work (three days/week), supporting the Medical Director of Provider Performance on clinical chart review, documentation improvement, provider education, and development of educational resources to advance high-quality, value-based care. Ongoing clinical practice keeps this role's knowledge current and grounds provider education and quality improvement initiatives in real-world experience.
Essential responsibilities consist of, but not all inclusive:
• Provide direct patient care two days per week, including comprehensive assessment, diagnosis, treatment planning, and management of acute and chronic condition
• Review clinical charts for completeness, accuracy, and documentation quality to support high-quality patient care and value-based care standards
• Identify gaps in clinical documentation and appropriate problem identification to improve clinical clarity, care planning, and continuity of care
• Assist in developing and delivering provider education, using clinical chart reviews to identify documentation gaps and opportunities to improve clinical reasoning, problem identification, documentation quality, continuity of care, and value-based care practices
• Support the creation, updating, and maintenance of educational materials, clinical tools, workflows, and learning resources
• Support provider onboarding and training to promote clinical excellence, documentation quality, and value-based care
• Perform additional duties related to clinical education, documentation support, chart review, and provider development as assigned to support organizational goals
What You'll bring - Education Requirements
• Master's degree or higher in Nursing (Nurse Practitioner program) required
• Current NP licensure and board certification required
What You'll Bring - Experience Requirements
• 3+ years of clinical experience as a Nurse Practitioner, value-based care experience preferred
• Experience in primary care
• Strong background and passion for clinical documentation improvement and chart review
• Strong clinical assessment and decision-making skills
• Strong written and verbal communication skills, including presentation development and delivery
• Technologically proficient and comfortable working within EHRs, data platforms, virtual collaboration tools, and digital education systems
• Proficiency in Microsoft office platforms including word, excel, and power point
• Ability to travel to other centers and flex schedule based on operational needs
• Excellent team player and communication skills required
• Bilingual/Bicultural (English and Spanish)
• Experience in provider education or academic medicine preferred
• Experience developing clinical education materials preferred
• Knowledge of quality metrics in value-based care preferred
Physical Demands
While performing the duties of this job, the employee is required to sit for prolonged periods of time, stand, walk, talk, hear, and reach with hands and arms. Hand eye coordination is necessary to operate computers and other office equipment; must be able to operate a motor vehicle. When representing Suvida Healthcare at external events, this role may also be required to stand for prolonged periods of time, kneel, crouch, bend and lift, and able to move up to 30 pounds.
