WHO WE ARE LOOKING FOR:

Franciscan Physician Network is seeking a full-time Primary Care and Geriatric Medicine Nurse Practitioner or Physician Assistant to meet the growing demand for chronic disease services at our Franciscan Healthy Living Center in Lafayette, Indiana. Provider will work in our Transition Clinic focusing on chronic disease management and will be the sole provider onsite during shifts. Advanced practice provider will lead a team-based care model that consists of respiratory therapists, dieticians, clinical nurses and social workers. The Healthy Living Center provides services that focus on chronic disease, diabetes and weight loss.

WHAT WE NEED:

Ideal candidate will have at least two years' experience in primary or urgent care, strong focus on clinical excellence, and patient satisfaction. Candidate will display exceptional interpersonal skills and desire to develop strong ties with the community and existing medical staff. Provider will need to work collaboratively with the care team to coordinate and deliver the highest quality of patient care. Provider must be very team focused and have excellent leadership qualities as you will be collecting information from the group, assessing the care plan and making the final determination of the course of action.

WHAT YOU CAN EXPECT:
  • Clinic hours: Monday - Friday, 8:00 a.m. - 5:00 p.m.
  • Flexible full-time schedule
  • No call expectation
  • Average outpatient volume: 10 patients per day
  • No collaborating physician on site, will be sole provider leading the care team
  • Responsibilities include physical assessment, ordering medications, following guideline therapies, helping transition from hospital to home, educating on disease process, outreach to patients and coordinating care with primary, specialists and other programs like home care, paramedicine
  • Potential outreach to satellite office located in Crawfordsville, IN
