$57.79 - $79.76 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights

  • Location: Glenbrook Ambulatory Care Center, Glenview, IL
  • Full Time/Part Time: Full-Time (average of 40 hours/week)
  • Hours: Monday through Friday between 8:00AM – 4:30PM
  • Required Travel: Evanston and Highland Park
  • Outpatient role

Job Description

Exciting opportunity to join Endeavor Health with the Research Institute – Urology – Precision Medicine Team! The Research – Advanced Practice Provider (APP) leads a novel, interdisciplinary precision medicine clinic operationalizing Polygenic Risk Scores (PRS) for patients identified through the Genomic Health Initiative (GHI). The APP evaluates high-risk patients (PRS >1.5) for conditions including prostate cancer, cardiovascular disease (CVD), and colorectal cancer, combining clinical expertise in genomics with care coordination to deliver individualized counseling, risk-stratified diagnostic testing, and specialist referrals. The APP manages clinic workflows, supports research data collection, and contributes to demonstrating the clinical and financial sustainability of a genomics-driven preventive care model.

  • Conduct initial and follow-up patient visits to review PRS results and their implications for disease susceptibility
  • Review medical and screening histories to identify gaps in preventive care
  • Order appropriate diagnostic tests based on PRS findings (i.e.: PHI, CAC, colonoscopy)
  • Manage follow-up on test results and initiate guideline-directed therapy (e.g., statins, lifestyle counseling, urology referral) as indicated
  • Collaborate with domain experts in Urology, Cardiology, and Gastroenterology, to manage actionable findings
  • Serve as a clinical resource for protocol interpretation and medical decision-making
  • Communicate participant safety issues and clinical concerns to the supervising physician and research team
  • Participate in team meetings, institutional events, and sponsor or department conferences
  • Manage clinic workflows, patient scheduling, and recruitment of eligible GHI participants
  • Maintain regulatory compliance with institutional SOPs and IRB requirements
  • Administer pre- and post-visit patient questionnaires to assess PRS understanding and perceived utility
  • Facilitate data collection for clinical and financial analysis using REDCap or other approved data capture systems

Qualifications:

  • License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
  • Education: Completion of Master’s level program in specialty area
  • Certification: BLS/CPR certification required
  • Experience: 0–3+ years of clinical experience in primary care, oncology, cardiology, or a related specialty

Special Knowledge and Skills Required:

  • Excellent organizational, documentation, and time-management skills supporting simultaneous clinical and research responsibilities
  • Strong collaborative skills across a multidisciplinary team of specialists, researchers, and administrators

Preferred Qualifications:

  • Interest in or experience with translational genomics, precision medicine, or genetic counseling
  • Strong interpersonal and communication skills, with the ability to explain complex genetic risk data and actionable patient education, in plain language to a lay patient audience
  • Ability to work independently in a nurse practitioner-led environment while coordinating with a multidisciplinary team
