Nurse Practitioner (NP) or Physician Assistant (PA) to provide comprehensive, patient-centered primary care in a collaborative, multidisciplinary environment.
Key Responsibilities
- Deliver comprehensive primary care to adult patients with complex medical and behavioral health needs.
- Develop and manage individualized care plans focused on patient goals and outcomes.
- Coordinate care across healthcare settings and community resources.
- Educate and counsel patients and families using evidence-based teaching strategies and teach-back methods.
- Collaborate with physicians, care managers, and other healthcare professionals to optimize patient care.
- Participate in quality improvement, population health, and value-based care initiatives.
- Maintain accurate and timely documentation within the electronic health record (EHR).
- Support onboarding and precepting of APCs, students, and other learners as needed.
- Promote an exceptional patient experience through compassionate, relationship-based care.
Qualifications- Graduate of an accredited Nurse Practitioner (Adult, Adult-Gerontology, or Family Medicine) or Physician Assistant program.
- Minimum 3 years of experience in primary care, internal medicine, family medicine, or complex care management.
- Current Delaware licensure and board certification as:
- Advanced Practice Registered Nurse (APRN), or Physician Assistant (PA-C)

This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
