SCHEDULE: Full Time; Monday- Friday, on call hours required

PAY RANGE:

  • Degree In Progress: $73,216.00 annually
  • BBS Registered: $78,104 to .$82,492.80 annually
  • LMFT/LPCC/LCSW: $91,374.40 to $111.675.30 annually

JOB SUMMARY/OVERVIEW: (The facilitator initiates parent and youth engagement, Child and Family Team (CFT) formation, the development of safety plans and an initial individualized CFT plan, always maintaining a non-judgmental, non-biased, strength-based stance. They coordinate and ensure initial and revised plans that reflect the youth and family's prioritized needs and preferences, with refinement of plan strategies learning from what did and did not work. They introduce and plan for transitions from the beginning of care. Consistently demonstrating Wraparound values and principles, the facilitator provides leadership, engaging others in a shared team mission, and displaying strong interpersonal competency. They understand the complex systems of public services and can forge strong connections with the members of the CFT, demonstrating respect for the power of divergent perspectives in support of Wraparound success.

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING

  1. Experience/Lived Experience:


A minimum of two years experience working with youth involved in the Child Welfare and/or Juvenile Justice System in behavioral health field required. Previous experience providing Wraparound services preferred.

  1. Special Skills/Training:


  • Bilingual Spanish/English strongly preferred.
  • Must provide proof of current driver's license and auto insurance.
  • BLS Certification required.


ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

1.

Conducts screening for clients and their families who are involved with the Child Welfare and/or Juvenile Justice system referred to the Wraparound Leadership team.

2.

Provides youth and families new to the wraparound process with information about the process and procedures to assist them to successfully engage.

3.

Facilitates initial strengths, needs, and cultural discovery process for families referred for Wraparound.

4.

Convenes and facilitates Child and Family Team meetings with child, family, natural supports and formal supports/professionals.

5.

Works with child and family to develop safety plan and individualized child and family plan.

6.

Utilizes evidence-based assessment and outcome measurements tools, compiles and reports data.

7.

Completes all client information and documentation in a timely manner (48 hours) according to guidelines.

8.

Maintains a 65%+ level of productivity of direct client service.

9.

Works with clients to establish long and short-term goals. Facilitator is responsible for detecting emotional, psychological, and physical danger signals. Facilitator is non-judgmental and offers feedback and support to clients.

Responsible for case management, crisis intervention, safety planning and collateral services for youth.

10.

Provides information to youth and families including treatment alternatives and referral service.

11.

Responsible for remaining within the ethical standards established in the field, including those of confidentiality.

12.

Attend clinical supervision and weekly staff meetings. Attend training/conferences as arranged.

13.

Works as a part of a multi-disciplinary team including Probation, Juvenile Detention Facility staff, CWS, behavioral health clinicians, Youth and Family Specialists, youth and parent partners, and other county agencies to coordinate care for youth and families as directed by supervisor.

14.

Responsible for maintaining the standards as listed in the CommuniCareOLE Personnel Policies and Procedures Manual, and any other standards which may apply to this position.

15.

Performs other duties as assigned.
