About the RoleWe are looking for a Physical Therapist to serve as our Virtual Occupational Health Physical Therapist (Evaluation-Only), supporting employer-partner populations across the full spectrum of job types, from large-scale industrial and warehouse/fulfillment roles to corporate and office-based staff, with single-encounter, virtual musculoskeletal evaluations. This role is built around two core outcomes: routing every employee to the right next-step care pathway, and clearly characterizing the disabling condition to support the accommodation process. The evaluating PT assesses the employee's presentation, determines work-relatedness and injury severity, identifies red and yellow flags, and then translates that assessment into (1) a specific care pathway decision and (2) a clear clinical description of the employee's functional limitations, which the employer's accommodation team uses to determine and implement any duty modification. The PT does not make the accommodation decision itself; the role is to provide the clinical basis the accommodation team needs to act on. This individual will not carry a treatment caseload; instead, they hand off every case to the appropriate downstream pathway (self-directed home program, virtual PT episode of care, in-person specialist referral, or workers' compensation coordination) and close the loop with Care Coordinators. He or she will report to the VP of Care Delivery.
What You'll Do- Conduct single-visit virtual evaluations for employees reporting work-related musculoskeletal symptoms, across a broad range of job types (physically demanding/industrial roles as well as sedentary/office-based roles), using a structured subjective history and virtual objective exam.
- Apply strong clinical reasoning to determine work-relatedness, injury acuity, and appropriate level of care within a virtual, evaluation-only model.
- Determine and initiate the correct next-step care pathway for every case (self-directed home program, virtual PT episode of care, in-person specialist referral, or workers' compensation coordination), making this routing decision the central output of the visit.
- Clearly characterize the disabling condition and resulting functional limitations through the clinical exam, and communicate findings to the employer's accommodation team to support their duty-modification decision, tailored to the physical demands of the employee's role (lifting/repetitive motion in an industrial setting or posture/ergonomics at a workstation).
- Identify red and yellow flags requiring urgent medical escalation and route accordingly.
- Generate concise, defensible documentation to support workplace accommodation requests, injury reporting, and payer or employer review.
- Communicate and coordinate with patients, Care Coordinators, and employer or payer partners to ensure a clean handoff and continuity across the episode, including direct communication with the employer partner's accommodation team member as needed to clarify findings or functional limitations.
- Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory, payer, and organizational requirements.
- Adhere to established clinical protocols, escalation pathways, and quality standards within a value-based, occupational health care environment.
Required Qualifications- Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited program.
- Active, unrestricted physical therapy license in the state(s) where care will be delivered; PT Compact State licensure strongly preferred.
- Minimum of 3 years of clinical experience, with preference for 5 years.
- Strong clinical reasoning skills and comfort making rapid, single-encounter evaluation and disposition decisions.
- Comfort practicing in a virtual/digital care delivery model, including remote evaluations and video-based patient education.
- Working knowledge of workplace accommodation, duty-modification, or return-to-work documentation.
Preferred Qualifications- Primary residence in a PT Compact member state with an active home-state license in good standing, allowing quick acquisition of Compact Privileges in other member states as needed.
- Experience in occupational health, industrial/workplace injury evaluation, or work conditioning.
- Experience evaluating both industrial/physically demanding roles and office/sedentary roles.
- Prior experience supporting an employer-partner or workers' compensation population.
- APTA specialist certification (e.g., OCS, SCS).
- Familiarity with functional capacity evaluation, ergonomics, or ADA/accommodation processes.
- Experience delivering care in virtual or digital health/telehealth-based models.
- Experience working in interdisciplinary or integrated care teams alongside medical providers.
