Role Overview: Nurse Practitioner, Neurology and Memory Care
We are seeking a skilled and compassionate Nurse Practitioner who is licensed in Connecticut to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.

Job Description • Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth • Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies • Partner with our neurologist(s) and broader care team to ensure continuity of care • Educate patients and families on diagnoses, treatment plans, and disease progression • Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements
