Full Job Description
Job Summary:

The Advanced Practice Provider will work with the collaborating physician to provide care for Hematology/Oncology patients at the Upstate Cancer Center.

As an Upstate Cancer Center APP you will be responsible for:

Comprehensive health assessments, conducting follow-up visits.

Initiating, and completing treatment plans and continuing chemotherapy treatments.

Managing the in-basket and ensuring quality care of this patient population.

Submitting prescription refills, disability paperwork, treatment plan signing and infusion coverage.

Ordering interventions based on clinical assessments.

Consult with physician for abnormal findings as appropriate and assist in planning and ordering care and treatment for patients requiring triage.

APP will actively participate in quality improvements working on teams and committees.

Minimum Qualifications:

NYS licensed/registered NP or PA required. Master's Degree as a Nurse Practitioner or Physician Assistant & current National Certification consistent to the clinical practice also required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.

CPR-BLS Certification required at the time of appointment.

Preferred Qualifications:

Prior experience in Hematology/Oncology.
