Full Job Description
Sola Academy by Fusion, an ISBE-approved therapeutic day school serving students with disabilities in grades 6-12, has an opening for a Transition Coordinator.

This position partners with students, families, and staff to support successful post-secondary outcomes, preparing students for college, careers, vocational training, and independent living through individualized transition planning, career exploration, community partnerships, and skill-building.

What You'll Do:
  • Develop and monitor transition goals within students' Individualized Education Programs (IEPs).
  • Ensure transition plans reflect each student's strengths, interests, preferences, and post-secondary goals.
  • Engage students in meaningful career exploration, college planning, and independent living preparation.
  • Support development of self-advocacy, executive functioning, decision-making, and workplace readiness skills.
  • Coordinate work-based learning opportunities, internships, job shadowing, and career exploration experiences.
  • Develop partnerships with employers, community organizations, vocational programs, colleges, and training providers.
  • Guide students and families through college, vocational, and alternative post-secondary pathways.
  • Assist students with college applications, scholarship opportunities, and admissions processes.
  • Develop individualized testing plans and coordinate accommodation requests for SAT, ACT, and other assessments.
  • Maintain familiarity with Common Application, Coalition Application, and state university admissions processes.
  • Collect and maintain records related to student testing, post-secondary plans, and college acceptances.
  • Attend IEP meetings and contribute to transition planning and goal development.
  • Collaborate with families, teachers, service providers, and community agencies to support student outcomes.
  • Maintain accurate documentation of transition activities and student progress.
  • Other duties as assigned.


What We're Looking For:
  • Bachelor's Degree required; Master's Degree preferred.
  • Experience in special education, school counseling, transition services, college counseling, vocational rehabilitation, or related field.
  • Knowledge of post-secondary planning, career readiness, and transition requirements under special education law.
  • Experience working with adolescents and young adults with disabilities.
  • Strong organizational and project management skills.
  • Expertise in transition planning and post-secondary pathways.
  • Ability to build partnerships with community organizations, colleges, and employers.
  • Strong relationship-building and communication skills.
  • Knowledge of college admissions, vocational programs, workforce development, and independent living resources.
  • Ability to motivate and support students in goal development and self-advocacy.
  • Excellent organization, documentation, and follow-through.

In addition to base salary, Fusion Heads of School are eligible for performance bonuses tied to KPI achievement.

Employment is contingent upon authorization to work in the United States. We do not provide visa sponsorship for this role.

Pay Range

USD $40.00 - USD $45.00 /Hr.
