Responsibilities

  • Serve as the lead contact and expert for the Care Coordination team.

  • Provide ongoing training for existing team members as needed.
  • Support onboarding and training for new employees effectively.
  • Motivate and guide the team to enhance performance and morale.
  • Manage a daily patient caseload to ensure proper care delivery.

Full Job Description
Care Coordination team is hiring a Care Coordination Team Leader.

Pay Range: $88,000.00 - $95,000.00 / annual

Schedule: Full-Time, Monday-Friday, 8:00am-4:30pmRequired to work 6 weekends per year.

Care Coordination Team Leader Responsibilities:
  • Acts as a lead point-of-contact and knowledge expert to all employees within department or assigned area.
  • Effectively coordinates staff scheduling, workflow, and delegation of daily tasks to ensure work is completed on a timely and accurate basis.
  • Notifies department leadership when there are issues with daily workflow and performance.
  • Identifies and resolves issues affecting the successful completion of delegated tasks and duties.
  • Provides on-going training to existing team members as required.
  • Acts as a resource during the on-boarding and training of new employees.
  • Provides support, guidance, leadership and motivation to promote maximum performance.
  • Maintains daily patient caseload.
  • Assume other duties as assigned by department leadership.

Care Coordination Team Leader Requirements:
  • Must be licensed as a Registered Nurse in New York State.
  • Associate's Degree required, Bachelor's Degree (BSN) preferred.
    • Bachelor's Degree required within 8 years of being hired for those licensed after 2017.
  • Three (3) to Five (5) acute hospital experience required.
  • Case Management, Utilization Management experience preferred.
  • Excellent oral and written communication and interpersonal skills.
  • Knowledge of federal and state regulations (DOH, Medicare/Medicare).
  • Strong organizational and problem solving skills.
  • Knowledge of third party payers and /or managed care principles.
