About the Role:Skilled Nursing Pharmacy in San Diego, CA is looking for a dedicated and knowledgeable Staff Pharmacist to join our team. This is an exciting opportunity to make a meaningful impact by providing expert pharmaceutical care to skilled nursing facility patients who depend on precise, compassionate medication management every day.
Responsibilities:- Dispense and verify medications accurately for skilled nursing facility (SNF) residents
- Perform drug utilization reviews (DUR) and ensure medication therapy appropriateness
- Collaborate with nursing staff, physicians, and care teams to optimize patient outcomes
- Monitor for drug interactions, contraindications, and adverse drug reactions
- Ensure compliance with state and federal pharmacy regulations and long-term care (LTC) guidelines
- Manage controlled substance handling, documentation, and reporting
- Support medication administration record (MAR) accuracy and pharmacist order review
Requirements:- Active California Pharmacist License (RPh) in good standing
- Doctor of Pharmacy (PharmD) degree from an accredited institution
- Experience in long-term care, skilled nursing, or institutional pharmacy preferred
- Strong knowledge of geriatric pharmacology and SNF regulatory requirements
- Familiarity with pharmacy dispensing software and electronic health records (EHR)
- Excellent attention to detail and strong clinical judgment
- Effective communication and team collaboration skills
Compensation: $61.00 - $65.00 per hour
