About the Role:

Skilled Nursing Pharmacy in San Diego, CA is looking for a dedicated and knowledgeable Staff Pharmacist to join our team. Responsibilities:
  • Dispense and verify medications accurately for skilled nursing facility (SNF) residents
  • Perform drug utilization reviews (DUR) and ensure medication therapy appropriateness
  • Collaborate with nursing staff, physicians, and care teams to optimize patient outcomes
  • Monitor for drug interactions, contraindications, and adverse drug reactions
  • Ensure compliance with state and federal pharmacy regulations and long-term care (LTC) guidelines
  • Manage controlled substance handling, documentation, and reporting
  • Support medication administration record (MAR) accuracy and pharmacist order review

Requirements:
  • Active California Pharmacist License (RPh) in good standing
  • Doctor of Pharmacy (PharmD) degree from an accredited institution
  • Experience in long-term care, skilled nursing, or institutional pharmacy preferred
  • Strong knowledge of geriatric pharmacology and SNF regulatory requirements
  • Familiarity with pharmacy dispensing software and electronic health records (EHR)
  • Excellent attention to detail and strong clinical judgment
  • Effective communication and team collaboration skills

Compensation: $61.00 - $65.00 per hour
