JOB SUMMARY

Provide professional skills in the distribution of drugs and in clinical services offered to patients and staff.

JOB DUTIES AND RESPONSIBILITIES:

Job Specific:
  1. Interprets physician's orders for medications and ensures therapy is appropriate for patient's diagnosis, weight, and age. Verified patient's allergy information. (EF)
  2. Compounds and dispenses prescribed medications per physician's orders in all patient care areas in accordance with departmental policies and applicable laws. (EF)
  3. Verifies and transcribes physician orders within the hospital information system thereby maintaining patient medication profiles. (EF)
  4. Verifies outpatient prescriptions and counsels patients receiving medications. (EF)
  5. Provides pharmaceutical care to patients through the use of Vancomycin dosing guidelines, aminoglycoside guidelines, antimicrobial stewardship program as well as review current laboratory data for pharmacokinetic dosing and medication levels and make appropriate recommendations to physicians.   6. Documents all clinical interventions in the patient's profile. (EF)
  7. Detects, prevents, evaluates, and reports Adverse Drug Events. (EF)
  8. Promotes the use of the formulary by converting non-formulary orders to formulary when possible. Coordinates procurement of non-formulary drugs when necessary. (EF)
  9. Insures proper technique and accurate preparation of all pharmaceutical products, including oral, IV admixtures, chemotherapeutic, and investigational agents. (EF)
  10. Insures compliance with controlled substance distribution according to all state and federal regulation. (EF)
  11. Participates in medical emergencies. (EF)
  12. Provides drug information to members of the health care team. (EF)
  13. Supervises the activities of supportive personnel. (EF).
  14. Assists in the training and development of newly employed departmental staff. (EF).
  15. Participates in department specific Performance Improvement Activities. (EF)
  16. Performs related duties as assigned or requested. (EF)
  17. Is able to practice in a variety of areas within the pharmacy as required by workload. (EF)
  18. Responsible for the most cost effective compounding and dispensing of medications. (EF)
  19. Follows all Texas State Board of Pharmacy Rules and Regulations, Citizens Medical Center Pharmacy Policies and Procedures, USP 797 and NIAHO standards. (EF)

Hospital:
  1. Adheres to organization-wide and other applicable policies and procedures.
  2. Day-to-day performance complies with the hospital's Service Excellence Behavioral Expectations.
  3. Performs within the prescribed limits of the hospital's Ethics, Compliance and Confidentiality Program guidelines.
  4. Performs within professional Scope of Practice at all times.

Minimum Requirements

EDUCATIONAL REQUIREMENTS:

Required: Bachelor's Degree in Pharmacy or Pharm D. degree from an accredited school of pharmacy.

EXPERIENCE:

Preferred: 2 Years' Hospital Pharmacy Experience

LICENSURE / CERTIFICATION REQUIREMENTS:

Required: Current Texas Pharmacy License

Preferred: Preceptor license from the State Board of Pharmacy; Sterile Compounding Certification
