No weekends or overtimePosition Summary:We are seeking a licensed
Speech Language Pathologist to evaluate and treat residents/patients with speech, hearing, swallowing, and cognitive challenges. This role involves providing therapy, developing treatment plans, and collaborating with physicians, rehab staff, and nursing teams. Entry-level candidates are welcome to apply.
Requirements:- Bachelor's degree in Speech Language Pathology
- Active Arizona SLP license
- TB test (2nd Step or QuantiFERON Gold)
- CPR/BLS Certification
Key Responsibilities:- Assess patients and create individualized therapy plans
- Provide direct therapy and monitor progress
- Educate residents/patients and families on therapy techniques
- Maintain accurate documentation and comply with HIPAA
- Collaborate with interdisciplinary teams
Compliance Items Needed:Resume, Level 1 Fingerprint Clearance, OIG/SAM Check, CPR/BLS Card, TB Test, License/Certification, Hepatitis B & COVID vaccination or declinations, Agency Skills Assessment Packet
