Guided by our core values of Passion, Respect, and Excellence, the Social Worker/Case Manager supports residents and families through individualized care planning, advocacy, and access to resources throughout their stay and transition to the next level of care.
Responsibilities for Social Worker Case Manager:
• Assess residents' psychosocial needs, strengths, goals, and barriers to care.
• Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
• Provide emotional support, counseling, and resource referrals to residents and families.
• Advocate for residents, ensuring their needs, preferences, and concerns are communicated and respected.
• Maintain communication among residents, families, physicians, clinical departments, and external providers.
• Participate in care planning meetings and monitor progress as residents' needs change.
• Coordinate discharge planning, referrals, appointments, transportation, and community services to support continuity of care.
• Educate residents and families about available services, care plans, and transition options.
• Maintain accurate documentation and comply with facility policies, regulations, and professional standards.
Qualifications for Social Worker Case Manager:
• Bachelor's degree in social work or a related field required; master's degree strongly preferred.
• Minimum of one year of social work or case management experience in a healthcare setting required; nursing home or rehabilitation experience strongly preferred.
• Knowledge of regulations, policies, and guidelines governing skilled nursing facilities.
• Strong understanding of care coordination, discharge planning, and community resources.
• Excellent communication, organization, and problem-solving skills.
• Ability to interact tactfully with residents, families, staff, and external agencies.
• Ability to work full-time hours, maintain a flexible schedule, and collaborate effectively in a fast-paced healthcare environment.
