Physical Therapist (PT) – Home Health Signature Healthcare at Home (A Pennant Group Company)
📍 Location: Lincoln City, OR – Oregon Coast
🧑⚕️ Position SummaryAs a Home Health Physical Therapist, you will provide personalized, one-on-one care to patients in their homes—helping them regain mobility, independence, and quality of life.
This is a field-based role offering flexibility, autonomy, and the opportunity to build meaningful patient relationships.
✅ Key ResponsibilitiesPatient Care- Evaluate and treat patients recovering from injury, illness, or surgery
- Develop and implement individualized plan of care
- Improve patient mobility, strength, balance, and functional independence
- Provide interventions focused on fall prevention and safety
Care Coordination- Collaborate with physicians, nurses, and interdisciplinary team members
- Adjust treatment plans based on patient progress
- Participate in case conferences and care planning
Patient & Caregiver Education- Teach patients and families exercises, safety strategies, and mobility techniques
- Empower patients to manage their conditions at home
Documentation & Compliance- Complete accurate and timely clinical documentation (OASIS experience preferred)
- Follow all Medicare home health regulations and Oregon state requirements
Quality & Outcomes- Support improved patient outcomes and satisfaction
- Reduce rehospitalizations and promote safe recovery at home
🎯 QualificationsRequired- Licensed Physical Therapist (PT) in Oregon (or eligible)
- Graduate of an accredited Physical Therapy program
- Valid driver’s license and reliable transportation
- Ability to work independently in a field-based setting
Preferred- Prior home health experience
- Familiarity with OASIS and Medicare documentation
- Experience serving rural or geographically spread communities

💰 Compensation & Benefits (Oregon Coast Market)Compensation- Salary Range: $55 – $65 Hourly (DOE)
- Sign On Bonus: $10,000
