Job Description:
Reviews reports on Current Core Measure Hospitalizations and Re-admissions daily.
Coordinates the Readmission Prevention Committee Meetings, identifies recent readmission cases, reviews for improvement opportunities, creates and sends out meeting agenda, and completes meeting minutes.
Participates in in-patient visits and post-discharge phone calls, utilizing data reports to identify patients with Core Measure diagnoses.
Visits patients in the hospital, provides education on disease process and medications, and collaborates with Care Management to address discharge needs.
Collaborates with Respiratory Therapy and Cardiopulmonary Rehab Teams when appropriate.
Collaborates with multidisciplinary team to reduce readmission rates.
