Starting Salary: RN/BSN $35.08 per hour - Full Salary for position $35.08-$47.69 per hour
RN-AD $33.36 per hour - Full Salary for position $33.36-$49.60 per hour

MINIMUM REQUIREMENTS- Bachelor's degree is preferred
- RN MN License
- 1-3 years of related work experience
- Valid driver's license

- Conduct comprehensive assessments of aging and medically fragile individuals to identify health, safety, and service needs.
- Develop, implement, and monitor individualized care plans, providing ongoing case management and coordinating services with healthcare providers, families, and community resources.
- Perform other Public Health nursing duties and related responsibilities as assigned.
