Job Title: Registered Nurse

Department: Home Care

Location: ElderOne -Seneca Towers, 200 Seth Green Drive Rochester, NY 14621

Hours Per Week: 40 hours

Schedule: Monday - Friday 8am -4:30-pm, 1 weekend per quarter.

RESPONSIBILITIES

• Patient Care. Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care;
• Communication. Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problems
• Evaluation & Documentation. Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff

• REQUIRED QUALIFICATION: Nursing Diploma or ASN, Registered RN in State of NY, NY driver's License

PREFERRED QUALIFICATION: BSN
1 year of experience working with a frail or elderly population preferred.Availability of Motor Vehicle6 months- 1 year experience conducting UAS-NY Community Assessment preferred.

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

PAY RANGE:

$75,733.00 - $101,014.00Rochester14621

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
