- RN: $99,840/yr

Schedule: Full-time Monday-Friday
Location: Field-based, Detroit metro. Branch office in Southfield, MI
Reports to: Clinical Manager / Branch Director / Director of Clinical Education
Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across the US, and we launched our Detroit branch this year. Our ultimate mission is to dramatically improve patient access to home health care.
The Field Supervisor role is how we make that mission scale: you guide newly hired and transitioning clinicians through real-world home health practice, validate their clinical competency, and model the standard of care every patient deserves.
What You Will Do- Time allocation: 75% field work (independently or with new joiners), 15% dedicated education time at your local branch, 10% special projects (e.g., Adaptive focus groups, improving education resources)
- Precept and coach newly hired clinicians during patient visits, providing real-time clinical guidance and feedback
- Provide hands-on mentorship and support to new joiners - with particular focus on clinicians with no prior home health experience
- Evaluate and sign off on new joiners' clinical competency through structured observation during patient visits
- Assist with skills lab training sessions and other structured onboarding activities
- Administer and support end-of-orientation assessments for new hires
- Serve as a resource and primary point of contact for new joiners throughout their orientation period
- Maintain a reduced but active patient caseload to stay current with clinical practice while dedicating meaningful time to precepting and teaching
- Maintain quality standards, including
- Model best practices in OASIS documentation, clinical decision-making, and adherence to Conditions of Participation (CoP)
- Identify and escalate concerns related to new joiner readiness or patient safety to the Clinical Manager
- Participate in Adaptive improvement focus groups, contributing frontline clinical insight to refine and enhance Adaptive's tools and workflows
Why Adaptive- A multiplier role: You'll shape every clinician who joins this branch - your impact compounds in a way direct patient care alone can't.
- Room to grow: We want to emphasize again how heavily we believe in promoting from within. If someone on the team is inclined towards leadership and is demonstrating that they can develop other clinicians, we're invested in exploring that!
- Stable salaried comp: Predictable annualized pay, so your move doesn't come with comp uncertainty.
- Real organizational influence: You'll participate in Adaptive focus groups that drive product decisions affecting every field clinician.
- Investment in your growth: We cover COS-C training and exam fees, provide an annual education fund, and pay for an Infusion Nurse Membership.
- Cutting-edge tech built for clinicians: Charting tools designed to reduce documentation burden; scheduling product launching soon.
- Mission-driven work: We are here to expand access to high-quality home health care, and we want clinical leaders who take pride in raising the standard.
