Department: Oncology and Main Pharmacy
Shift/schedule: Full Time (40 hrs/wk)

GENERAL SUMMARY

Works under the supervision of the Oncology Lead Pharmacist and Director of Cancer Services. Day-to-day Cancer Center Pharmacist duties are supervised by the Registered Pharmacist Oncology Lead. The primary job function of the Registered Pharmacist Oncology is to distribute medications and supplies according to departmental procedures with the assistance of pharmacy technicians. Collaborates with providers and staff on medication related matters. Educates patients, families, and/or providers.

QUALIFICATIONS

Education:

• High School Diploma or successful completion of an equivalent High School Exam Required
• Bachelor's degree in Pharmacy (5 years) required
• Doctor of Pharmacy (Pharm D) preferred

Licensure:

• Pharmacy(OH) license required
• Graduate Pharmacists may also be hired awaiting license
• BLS required
• Board Certification in Oncology is preferred

Experience:

• Previous hospital experience preferred

Interpersonal Skills:

• Must be able to communicate effectively with hospital, physicians, and nursing personnel when clarifying medication orders and dispensing pharmaceutical supplies.
• Must be able to provide drug information to a variety of clinical personnel.
• Must assign work and provide guidance to departmental personnel.

Essential Technical/Motor Skills:

• Inputting data/typing, speaking clearly, answering telephones, precise eye/hand/foot coordination, fingering/fine dexterity, delicate/precise manipulation, handling/gripping.

JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS

The following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time.

1. Compounds and dispenses medications and pharmaceutical supplies according to accepted standards of practice to fill written prescriptions issued by qualified prescribers.
2. Checks the accuracy and appropriateness of all medications dispensed and intervenes clinically when necessary related to optimizing drug therapy. Maintains accurate and appropriate patient profiling and charging records for all medication orders.
3. Follows accepted standards of practice and adheres to federal, state, and local regulations, as well as the policies and procedures of SOMC Cancer Center and Pharmacy Services.
4. Serves as a drug information source for patients, physicians, nursing personnel, hospital staff, visitors and the community.
5. Assists the Director of Pharmacy and Director of Cancer Center with certain assigned reports or projects.
6. Maintains a level of professional competency and remains current in the field of pharmacy including hematology and oncology.
7. Assigns, teaches, supervises, and monitors the work of Pharmacy Technicians, students, interns and externs.
8. Continuously evaluates pharmacy's involvement in the delivery of oncology/hematology care.
9. Educates clinical team members on oncology/hematology medications and trains and/or assists with rotating pharmacists in Meditech, MOSAIQ, and oncology/hematology work flows.
10. Performs advanced clinical duties that involve building oncology/hematology care plans, managing patient-related tasks, and oral oncology prescriptions.
11. Serves as preceptor for APPE pharmacy students/residents as requested.
12. Coordinates with Registered Oncology Pharmacist Lead and technicians for ordering, storage and handling of medications including chemotherapy, hazardous, and non-hazardous drugs and supplies.
13. Compounds and dispenses medication and pharmaceutical supplies according to accepted standards of practice, including adherence to safety requirements in the handling and preparation of chemotherapy, hazardous, and non-hazardous agents according to current guidelines (USP 797, USP 800, NIOSH) to fill prescriptions issued by qualified prescribers.
14. Intervenes clinically by assisting with clinical monitoring by laboratory assessment of appropriate CBC, renal, hepatic, electrolyte and other pertinent measurements and patient specific criteria as recommended for safe delivery of chemotherapy, hazardous, and non-hazardous agents when necessary and appropriate to optimize drug therapy.
15. Participates in identification and reporting of therapy errors, adverse drug reactions or other related near miss opportunities related to safe medication supply and delivery.
16. May educate patients and their families to ensure the understanding of the goal, administration schedule, toxicity management of chemotherapy, immunotherapy, targeted therapy, and hematology treatments and documents the activity and interventions in the EMR.
17. Supports budget targets through inventory and formulary items with appropriate charge codes and waste modifiers.
18. Maintains a level of professional competency by completing at least 10 hours of CE annually related to oncology or hematology.
