Full Job Description
Responsibilities:
  • Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client's unique needs
  • When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
  • Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements:
  • Current Certificate of Competence with the College of Nurses of Ontario.
  • Availability to work at all 4 clinics within WW and SW SDC.
  • Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
  • A willingness to attain required SE Health certifications
  • A current CPR certificate is required.

AI and compensation details:

We use AI to take notes during our interview. All applications and interviews are reviewed by our Talent Acquisition team. This role is a new addition. The hiring pay range is $37.35-41.35 (per hour), based on experience.
