Qualifications- Active New York State RN or LPN License
- Current CPR Certification
- Strong communication and patient care skills

About the RoleAs a home care nurse with Serene, you will provide skilled, individualized nursing care to patients in their homes throughout Suffolk County.
Responsibilities may include:
- Providing direct nursing care based on individualized patient care plans
- Monitoring patient conditions and documenting care
- Administering medications and treatments as prescribed and within your scope of practice
- Communicating with families, physicians, and clinical supervisors
- Supporting patients with their daily healthcare needs
