Description

Coverage Territory: Bloomington, Pontiac, Streator, Seneca, Ottawa, LaSalle Peru, Dwight, Marseilles, and surrounding areas.

Schedule: 8 PM-8 AM, alternating 4-day and 3-day weeks including weekends in rotation

What You'll Do

As a Transitions Hospice Night Nurse, you'll serve as a critical support member of the interdisciplinary care team during overnight hours - assessing, treating, and communicating patient needs with clinical confidence and genuine compassion. Your responsibilities include:
  • Delivering professional nursing care across the full nursing process within your scope of practice
  • Conducting thorough patient and family assessments and contributing to individualized care plans
  • Completing all visit types as needed to meet overnight patient and family needs
  • Administering medications and treatments per physician orders
  • Identifying nursing diagnoses from health assessment data and responding with sound clinical judgment
  • Recording accurate, timely clinical notes including pain and symptom management outcomes
  • Communicating changes in patient condition to physicians and obtaining orders as required
  • Coordinating with community health partners to support continuity of care
  • Ensuring equipment and necessary services are in place for patients
  • Maintaining and growing your clinical skills through continuing education and in-service participation
