Initiates and completes patient assessments throughout the continuum of care (admission to discharge); identify physical, social and psychological response of patient and family using interviewing and physical assessment skills. Modify standard of nursing care to reflect needs and changes in patient status; sets reasonable goals and designates intervention to treat identified problems/needs. Use professional nursing judgment regarding the patient's condition in relation to physician's orders and protocols and administration of medications and treatments, optimally upholding a safe patient environment and clinical practices. Implement the plan of care utilizing nursing interventions which reflect an understanding of the nursing process, standards of practice and standards of care. Provide individualized patient care in a manner which demonstrates clinical knowledge, technical competence and knowledge of the patient population, age-specific behavior, growth and development for the scope of service; documenting information relevant to patient status and response to care. Continuously evaluate patient's responses to medical regimen and nursing interventions, modifying plan of care and recognizing and taking appropriate action for significant changes in patient conditions. Ensures patient and family involvement with discharge planning and the achievement of identified needs through teaching or referrals. Assumes responsibility for ensuring an integrated discharge planning process. Administer intravenous therapies in accordance with department scope of service and Medical Center and department policies. Function within the boundaries of hospital policy and safety, infection control procedures and standards and the Scope of Nursing Practice as defined by the New Hampshire Nurse Practice Act, and with respect to the Patient's Bill of Rights. Is accountable for appropriate use of materials and resources in planning and delivering care. Identify and offer solutions to patient care delivery issues and quality improvement by participating in meetings, in service and self-development education programs and activities. Treat each patient, family, visitor and employee with compassion and courtesy; maintaining excellent customer relations, a positive attitude, and an ability to work with others in a cooperative effort to ensure customer satisfaction and Continuous Quality Improvement. Promote the philosophy, value and goals of the organization and department and maintain a functional communication network for the promotion of effective customer service and culture of patient safety. May be required to perform other duties of lesser, equal or greater levels of responsibility.
Qualifications: Graduate of an accredited nursing program. Current NH RN license and BLS certification required. Previous critical care experience required. Electrophysiology experience preferred. ACLS required within 1-year of hire and maintained on a biannual basis. CCRN preferred, but not required. . Must have excellent communication and interpersonal skills and the ability to adapt to stressful situations quickly and effectively.
