Job Description:

  • Formulates individualized care plans based on therapeutic principles, evaluates the effectiveness of interventions, and modifies plans as needed to achieve desired patient outcomes.
  • Initiates patient coordination at the entry point into the system and prepares patient cases for pre-procedural review with a multidisciplinary team.
  • Navigates neuro medical inpatients from admission through treatment and discharge, ensuring timely medical and supportive services.
  • Assists in program development and expansion, actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.   • Serves as a patient advocate, ensuring patient rights and needs are respected and addressed.
  • Maintains compliance with regulatory standards and organizational policies, ensuring legal and ethical practice.
