Current State or Compact Registered Nurse (RN) License.
Minimum one year of clinical experience preferred.
Knowledge in Oasis and/or end of life care preferred.
Demonstrated knowledge in current nursing practices.
Valid state driver's license and automobile liability insurance required.

We are looking for full time, part time orPRN RNs who are passionate about providing quality home health care to our community-based patients in Granbury, Weatherford,TX and the surrounding areas.
Requirements
Key Responsibilities:
Follow a designated care plan in accordance with patient needs.
Make home visits to clients in designated geographic territories.
Perform assigned duties, including medication administration, wound care, treatments, and procedures.
Monitor client conditions and report changes to the Clinical or Client Services Manager.
Follow up with, execute, and properly document doctors' orders.
Perform client assessments.
Provide case management and coordination of care.
Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
