Skilled Nursing Visit Rates- IV/Infusion Visit: $125 per case

Responsibilities- Conduct comprehensive patient assessments
- Develop and implement individualized care plans
- Administer medications, treatments, and nursing interventions
- Monitor patient conditions and communicate significant changes
- Provide skilled nursing services, including:
- Wound Care
- IV Therapy
- Catheter Care and Management
- Educate patients and families on:
- Disease Management
- Medication Compliance
- Home Safety Practices
- Maintain accurate and timely clinical documentation
- Ensure compliance with all state and federal regulations
- Provide compassionate care and emotional support to patients and families
Qualifications- Active New York State Registered Nurse (RN) License (Required)
- CPR/BLS Certification (Required)
- Minimum 1-2 years of nursing experience preferred
- Home Care, Geriatric, Hospice, or Related Clinical Experience Preferred
- Strong clinical assessment and critical thinking skills
- Ability to work independently while maintaining communication with the clinical team
- Excellent interpersonal and communication skills
- Experience with Electronic Health Records (EHR) preferred
- Valid Driver's License and Reliable Transportation Preferred
