Qualifications

• Current unrestricted RN license in the state of practice
• Preferred one year of clinical experience
• Preferred one year of home health experience
• Two-step negative TB skin test or chest x-ray required
• CPR certification required
• Level 1 Fingerprint Clearance Card required
• Valid driver's license and auto insurance required
• Transportation to meet job responsibilities required
• Understanding of regulatory compliance with state and federal regulations
• Adherence to HIPAA for client confidentiality

Responsibilities

• Provides care according to the state's nurse practice act
• Makes initial evaluation visits and regularly re-evaluates nursing needs
• Initiates and revises the plan of care as necessary
• Delivers specialized nursing services
• Implements preventive and rehabilitative nursing procedures
• Prepares clinical notes and summaries of care
• Maintains compliant clinical records
• Stays updated on nursing and home health care changes
• Promotes home health referrals within the healthcare community
