Registered Nurse Case Manager | VNA Home Health - Chelmsford, Lowell & Billerica Areas

Qualifications

• Compassionate and organized with strong multitasking abilities.
• Valid Massachusetts RN License in good standing.
• Minimum of 1 year of medical/surgical nursing experience.
• Acute care experience is a plus.
• Willingness to travel within designated territory.

Responsibilities

• Manage patient care plans and coordinate services for home health patients.
• Conduct assessments and develop individualized care strategies.
• Collaborate with healthcare teams to ensure comprehensive patient support.
• Educate patients and families on health management and resources.
• Document patient progress and maintain accurate records.
