Take on a clinical leadership role with real autonomy. As a Regional RN Care Coordinator, you'll be the clinical anchor for your region, reviewing every screening flag, coordinating complex care, and administering long-acting injectables, with a Medical Assistant team and RN Manager who have your back.
What You'll Do- Provide complex care coordination and clinical escalation for high-acuity clients across your region
- Build therapeutic relationships with clients facing substance use disorders and significant psychiatric mental health illness
- Review every flag from Medical Assistant screening and document a clear plan of action
- Administer long-acting injectable (LAI) medications and oversee medication refills
- Coordinate hospital discharge follow-up and confirm required CCBHC screening measures are completed
- Provide clinical oversight for Medical Assistants and share coverage through our distributed RN phone support model
What You'll Bring- Associate Degree in Nursing (ADN) required; BSN preferred
- 1-3 years of RN experience.
- Compassion and ability to connect and build rapport with clients facing challenges with SPMI
- Current, unencumbered Washington State RN license and current CPR/BLS certification
- Competency in long-acting injectable (LAI) medication administration
- Active drivers license with reliable transportation.
Nice to Have- BSN and 3+ years of RN experience and understanding of challenges clients with SUDS/SPMI face
- Prior experience in a CCBHC or community behavioral health setting
- Experience in a RN Case Management Role

Pay Range: $90,284 - $123,096 per year
