Full-Time Psychiatric Mental Health Nurse Practitioner- Baton Rouge, LouisianaHomedica is seeking a compassionate and skilled Full-Time Psychiatric Nurse Practitioner to deliver comprehensive mental health services across a variety of settings-including in-home visits, assisted living and nursing facilities, post-acute care facilities, and via telehealth.
Responsibilities:- Provide psychiatric services in home, facility, and telehealth settings
- Complete monthly psychotropic medication review, evaluate for and document gradual dose reductions in nursing facilities settings.
- Prescribe and manage medications and individualized care plans
- Coordinate care with interdisciplinary teams and community partners
- Perform evaluations and treat as appropriate.
- Participate in after-hours or on-call rotation as needed
Qualifications:- Active Nurse Practitioner license in Louisiana
- PMHNP Certification
- A minimum of 6 months prior work experience as a PMHNP
- DEA license required
- Strong communication, decision-making, and clinical skills
- Comfortable working independently across diverse care environments
Preferred:- Facility-based or post-acute care experience
- Experience in home health, hospice, or facility-based care
- Familiarity with telehealth technology and EMRs
