Program/Department Description Community Access provides affordable and supportive housing, health promotion services, and education and training to individuals living with mental health conditions, often also living with histories of homelessness, substance use, criminal justice involvement and serious health concerns. The agency is committed to assisting participants of services to acquire the knowledge, skills and self-assurance to improve and manage their own health.
Position OverviewThe Psychiatric Nurse Practitioner (PNP) will support participants and staff across programs within the Housing Division and be an integral member of the supportive housing teams. The position is flexible and adaptive, responding to changing programmatic and participant needs. Key responsibilities include psychiatric assessment, prescribing, and medication monitoring. The PNP coordinates care with other physicians including primary care, therapists and support workers, both internal and external to the organization. The PNP works collaboratively with program teams and other medical providers to ensure that care is coordinated and delivered within the framework of participant self-determination, recovery, and cultural competency.
Key Performance Indicators- Complete progress notes for all contacts, collaterals, and engagement attempts within 2 business days.
- Complete assigned psychiatric evaluations on time.
- Submit pharmacy prescriptions in a timely manner.
- Document changes in medication regime in AWARDS within 24 hours.
Job Qualifications- Have an understanding, appreciation, and commitment to the philosophy and mission of Community Access.
- Commitment to person-centered treatment strategies, upholding participants' rights, and self-determination in service provision.
- Current license in NYS as a Psychiatric Nurse Practitioner; NYS license as a Registered Nurse; Master's degree in advanced nursing from an accredited Nurse Practitioner Program.
- Minimum of three (3) years working with people with serious mental health conditions, co-occurring substance use disorders, and homelessness.
- Minimum of two (2) year working as a nurse practitioner, including prescribing preferred.
- Computer proficient
- Excellent written, verbal and interpersonal communication skills.
- Bilingual Spanish-speaking, preferred
- Must be fingerprinted and cleared by the New York State Justice Center.
