Job DescriptionOverview Hours: Monday-Friday, primarily days with occasional schedule variations based on operational needs. Potential hybrid opportunity, though approximately 90% of the role is on-site.
Purpose Statement/Position Summary: The Program Manager oversees the patient and family education program, with responsibility for planning, implementation, and evaluation of the patient/family education courses for patients across the care continuum Leads regular needs assessments to understand changes in patient populations and associated family learning needs and quality/safety trends for home-based care. Serves as a consultant to inpatient, ambulatory, and community settings to ensure continuity of care and programmatic offerings that minimize harm in the home setting and optimize patient outcomes and the patient/family experience. Leads continuous quality improvement initiatives to ensure that educational content is accessible, relevant, and evidence-based and uses a multimodal teaching approach to meet learning needs of a diverse population of patients and families. Tracks, analyzes, and reports program metrics to demonstrate impact and program sustainability.
Minimum Qualifications/Work Experience: Required :
- 5+ years' experience in nursing, with at least 3 years' experience in pediatrics.
- Strong organizational, data analysis, and project management skills
- Excellent verbal and written communication skills
Preferred :
- Experience with patient/family education
Education/Licensure/Certification: Required :
- RN prepared at the graduate level in nursing or a related field AND certified in area of specialty
- California RN Licensure
- American Heart Association BLS certification
Pay Scale Information $111,862.40-$149,136.00CHLA values the contribution each Team Member brings to our organization.
