Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments.
Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
Utilize Bloom's integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes.
Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.
