Full Job Description
In this role, you will serve as the primary clinical provider dedicated to a single facility, giving you the stability to build meaningful, long-term relationships with patients, families, and staff. This position is explicitly structured for work-life balance and clinical focus:
  • Monday-Friday, Day Shifts Only
  • Zero Night or Weekend Call
  • Single-Site Dedicated Focus (No driving between multiple facilities)

You will manage both acute and chronic conditions, guide post-hospital transitions, and partner closely with facility leadership to deliver compassionate, high-touch clinical care.

Key Responsibilities
  • Provide compassionate, evidence-based medical care to patients at Sadie G. Mays, including assessment, diagnosis, treatment, and ongoing management.
  • Direct complex chronic disease management and sub-acute rehab care, reducing unnecessary hospital readmissions.
  • Lead interdisciplinary care planning alongside facility nurses, therapists, social workers, and families to support patient-centered goals.
  • Support smooth transitions of care through post-admission evaluations, comprehensive medication reconciliations, and family conferences.
  • Complete timely, accurate clinical documentation in the facility EHR.
  • Serve as a clinical champion for quality improvement initiatives that directly enhance resident outcomes and facility care standards

Skills & Abilities
  • Strong clinical knowledge of adult and geriatric medicine, with expertise in chronic disease management.
  • Excellent communication and collaboration skills with patients, families, and facility staff.
  • Ability to manage a patient panel efficiently while maintaining a high standard of care.

Qualifications
  • Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Georgia.
  • National certification (AGNP, FNP, Adult, or PA-C) with an active, unrestricted DEA license.
  • Minimum 1-2 years of experience as an APP; strong preference for post-acute, skilled nursing (SNF), or geriatric care experience.
  • Mission-minded clinical approach with a genuine passion for senior care and health equity in community-based settings.
  • Strong collaborative communication skills to engage effectively with nursing staff, attending physicians, and resident families
