Requirements:
- M.D. (or foreign equivalent degree)
- Unrestricted license to practice medicine in Washington;
- Board certified or board eligible in Neurology;
- DEA, NPI & prescriptive authority;
- BLS for Healthcare Providers certification by the American Heart Association
