Job Description

  1. Evaluation - Obtains pertinent medical and social history. Performs thorough and accurate evaluations using techniques to objectively measure physical and functional status including those specific to specialty area. Determines functional and measurable short and long term goals and designs an appropriate treatment program. Considers discharge options at time of initial evaluation.


  1. Treatment - Performs treatment as outlined in the initial evaluation. Modifies treatment as necessary, updates goals and re-evaluates discharge plans throughout the course of treatment. Demonstrates thorough knowledge of treatment techniques and applies appropriately. Treatment should include patient and family education and communication/interaction with appropriate team members.


  1. Productivity- Productivity requirements and goals will be set forth by the department each fiscal year


  1. Documentation - Clinical documentation is timely, objective and complete. Administrative documentation including but not limited to billing and outcome measures is accurate and timely.


  1. Communication- Interacts with members of the inter-disciplinary staff pertaining to the welfare and management of assigned patients in a professional manner. Communicates with patients and family in respectful, professional & compassionate manner.
