Cortlandt Healthcare in Cortlandt Manor, NY is a 120 bed sub-acute facility with a full time Physical Therapist opening on our in house rehab team. Hiring is done on site by our Director of Rehab and facility leadership, not by an outside recruiter. You will interview with the people you will actually work with.
What Makes This PT Job Different- The whole team is in the room. PT, OT, SLP, nursing, dietitian, respiratory, social work, and MDS build one plan together. When a resident plateaus, you are not the only one looking at it.
- New graduates are trained, not thrown in. Structured orientation, a named mentor, a gradual caseload ramp, and ongoing in house education from clinicians who want to teach. If sub-acute is new to you, that is expected and planned for.

Your Day to Day- Complete physical therapy evaluations covering strength, range of motion, balance, gait, transfers, endurance, and fall risk
- Set functional, measurable goals tied to each resident's prior level of function and planned discharge setting, under physician direction
- Deliver skilled interventions including therapeutic exercise, gait and transfer training, balance and neuromuscular re education, and functional mobility training
- Complete evaluations, weekly progress notes, recertifications, and discharge documentation accurately and on time
- Train residents, families, and caregivers on safe mobility, transfers, assistive device use, and home carryover
- Partner with nursing on safe transfer status, fall prevention, and restorative programming
- Recommend assistive devices, wheelchairs, seating, and durable medical equipment for discharge
- Direct and supervise Physical Therapist Assistants and students in accordance with the state practice act
Requirements- Doctor of Physical Therapy, Master of Physical Therapy, or Bachelor of Physical Therapy from a CAPTE accredited program
- Valid New York Physical Therapist license, or license eligible
- Ability to direct and supervise Physical Therapist Assistants per the state practice act
- Sound clinical documentation across evaluation, weekly progress, recertification, and discharge
- Commitment to confidentiality, HIPAA, and Federal Resident Rights
Nice to Have- Sub-acute, skilled nursing, or geriatric rehabilitation experience
- Background in orthopedic post surgical, neurologic, or cardiopulmonary rehabilitation
New graduates and therapists moving from outpatient, acute, or home care are encouraged to apply. We will get you up to speed.
