Job Title: Home Health Physical Therapist - Santa Clara County

Location: South Bay, CA
Your Role- Independently perform physical therapy evaluations and reassessments
- Develop and manage individualized plans of care
- Deliver skilled therapy focused on mobility, function, fall prevention, and independence
- Collaborate with PTAs on an occasional, well-supported basis
- Communicate directly with physicians and the interdisciplinary team
- Educate patients and caregivers
- Complete documentation supported by AI tools
- Serve as the primary PT voice in patient care planning
Requirements- Current California Physical Therapist license
- Valid California driver's license
- Strong clinical judgment, organization, and time-management skills
- Home health experience preferred; strong outpatient, inpatient, or rehab PTs are encouraged to apply
