Physical Therapist (PT) - Full-Time

Kern River Transitional Care
Pay Range: $65.00-$70.00 per hour, based on experience

Kern River Transitional Care is seeking a compassionate and motivated Full-Time Physical Therapist to join our outstanding rehabilitation team. Essential Duties and Responsibilities
  • Evaluate residents' mobility, strength, balance, coordination, range of motion, and functional abilities.
  • Develop individualized treatment plans based on each resident's needs, goals, and medical condition.
  • Provide skilled physical therapy services to improve mobility, independence, safety, and overall function.
  • Help residents regain the ability to walk, transfer, maintain balance, and complete daily activities safely.
  • Establish measurable goals and modify treatment plans based on residents' progress and changing needs.
  • Provide therapeutic exercise, gait training, balance activities, transfer training, and other appropriate interventions.
  • Recommend and train residents in the safe use of walkers, wheelchairs, canes, and other adaptive equipment.
  • Educate residents, families, caregivers, and facility staff regarding treatment plans, safety, positioning, and mobility techniques.
  • Complete evaluations, progress notes, discharge summaries, and other required documentation accurately and on time.
  • Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other members of the interdisciplinary team.
  • Participate in resident care conferences, discharge planning, and quality-improvement activities.
  • Supervise Physical Therapist Assistants and rehabilitation support staff as permitted by law and facility policy.
  • Maintain residents' privacy and the confidentiality of all health information.
  • Follow all professional standards, facility policies, safety procedures, and regulatory requirements.
  • Perform additional duties as assigned.
Qualifications

  • Skilled nursing, rehabilitation, long-term-care, or geriatric experience preferred.

  • Ability to work independently and collaboratively as part of an interdisciplinary team.
  • Knowledge of PointClickCare or another electronic medical record system preferred.
