Compensation- $80-$85 per patient visit
- $200 per Start of Care (SOC) visit
- Full-Time Position
- Flexible scheduling and the opportunity to build your own caseload
What You'll Do- Provide skilled physical therapy services in patients' homes.
- Perform comprehensive initial evaluations, reassessments, and fall risk assessments.
- Develop individualized treatment plans to improve strength, mobility, balance, and functional independence.
- Educate patients and caregivers on home exercise programs and injury prevention.
- Collaborate with nurses, physicians, and the interdisciplinary care team to ensure quality patient outcomes.
- Complete accurate, timely, and compliant clinical documentation.
Qualifications- Current Hawaiʻi Physical Therapist (PT) license in good standing.
- Minimum of one year of clinical experience preferred.
- Home Health experience is a plus, but not required.
- Strong clinical judgment, communication, and documentation skills.
- Valid driver's license, reliable vehicle, and current auto insurance.
- Ability to travel throughout Oʻahu to provide in-home patient care.
