We are seeking a dedicated and compassionate Physical Therapist to join our team in a skilled nursing facility.
RESPONSIBILITIES:
- Conduct comprehensive evaluations of patients to identify physical, functional, and rehabilitation needs.
- Develop and implement individualized treatment plans in collaboration with the interdisciplinary team.
- Monitored and documented patient progress and adjusted treatment plans as necessary.
- Educate patients and caregivers on exercises and techniques to promote recovery and prevent future injury.
- Provide hands-on therapy, including exercises, manual therapy, and modalities as appropriate.
- Ensure compliance with regulatory standards and company policies in all aspects of care delivery.
- Participate in clinical meetings and contribute to a culture of continuous improvement.
QUALIFICATIONS:
- New passer/graduates are welcome
- With Skilled Nursing or Long-Term Care, experience is a plus
- Previous experience as a Physical Therapist (PT) is a plus
- Physical Therapy Degree from an accredited program
- State licensure or eligibility for licensure as a physical therapist
- Is in good standing with all regulatory agencies and licensing boards as required
- Interest in working with the geriatric population
- Ability to work effectively both independently and within a team
