Responsibilities:- Provide physical therapy services to patients in their homes and communities, including evaluation, treatment, and discharge planning
- Develop and implement individualized treatment plans to promote patient recovery and independence
- Collaborate with healthcare team members to ensure comprehensive care and optimal patient outcomes
- Participate in interdisciplinary team meetings and contribute to the development of patient care plans
- Conduct home visits and communicate effectively with patients, caregivers, and family members
- Maintain accurate and detailed records of patient care and progress
Requirements:- Current New Jersey Physical Therapist license
- Doctorate in Physical Therapy (DPT) or related field
- Strong clinical skills and knowledge of physical therapy techniques and best practices
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Strong organizational and time management skills
