We are seeking a licensed Physical Therapist to join a collaborative interdisciplinary care team providing rehabilitative services to medically complex and aging patients. The Physical Therapist will provide restorative and therapeutic care in both clinic and home-based settings, with a focus on maximizing patient safety, mobility, and independence.

Key Responsibilities:- Conduct Physical Therapy assessments in the clinic and in patient homes to identify care needs and develop treatment goals
- Participate in interdisciplinary team meetings to contribute to individualized care planning
- Deliver and document Physical Therapy services based on care plans and physician orders
- Administer treatments including active and passive exercises, gait and functional training, massage, and use of therapeutic modalities (heat, sound, light, water, etc.)
- Supervise Physical Therapy Assistants and provide guidance on Functional Maintenance Programs (FMPs)
- Educate patients, caregivers, and staff on therapeutic equipment and rehabilitation techniques
Requirements:- Bachelor's degree in Physical Therapy with at least five (5) years of experience required
- Master's or Doctorate in Physical Therapy preferred
- Valid California Physical Therapy license or license eligibility
- Two (2) years of experience working with elderly or medically complex patients in a clinical setting preferred
- Physical Therapy degrees obtained outside the U.S. must be evaluated to ensure U.S. equivalency
- Current CPR certification preferred
