Bring Outpatient Physical Therapy to the Patient.

What if you could provide the quality of care you value in an outpatient setting-while giving patients the convenience, comfort, and safety of receiving that care at home?

RVNAhealth is seeking experienced Physical Therapists to join our In-Home Outpatient (IHOP) team. This unique model brings outpatient-level physical therapy directly to patients in their homes and assisted living communities, helping them recover from surgery, restore mobility, and get back to the activities that matter most.

Our IHOP program works primarily with post-surgical and orthopedic patients, including individuals recovering from total joint replacements. You'll have the opportunity to provide meaningful, one-on-one care while working closely with patients, families, physicians, and other members of the care team.

  • South: Stamford / Greenwich and surrounding communities
  • Central: Ridgefield / Wilton / Redding and surrounding communities
  • North: Danbury / New Milford and surrounding communities
What You'll Do

As an IHOP Physical Therapist, you will:
  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
  • Provide outpatient-level PT services in patients' homes and assisted living facilities.
  • Treat primarily post-surgical and orthopedic patients, including total joint replacement patients.
  • Perform pre-operative safety evaluations and post-surgical follow-up visits.
  • Deliver evidence-based treatments consistent with Connecticut PT practice standards.
  • Develop and progress individualized home exercise programs.
  • Educate patients and caregivers on mobility, safety, exercises, and recovery.
  • Communicate and collaborate with physicians, families, and other members of the care team.
  • Identify when patients may benefit from additional home health services and make appropriate referrals.
  • Participate in case conferences and interdisciplinary team meetings.
  • Maintain accurate, timely clinical documentation and obtain physician orders as required.
What We're Looking For
  • Bachelor's degree or higher from an accredited Physical Therapy program.
  • Current Connecticut Physical Therapist license.
  • Minimum of 2 years of clinical experience.
  • Outpatient orthopedic, post-surgical, or home-based therapy experience preferred.
  • Familiarity with Medicare Part B regulations and billing for in-home outpatient services.
  • Strong clinical, communication, organizational, and time-management skills.
  • Ability to work independently while collaborating effectively with a multidisciplinary team.
  • Experience working in assisted living communities is a plus.
